Whitewater Families,

Whitewater Athletics will be returning this fall for the 2021 season. The sports offered will be Football, Cheer,  Volleyball, Golf,  and Softball. To be able to tryout, students must have a current physical ( Must be on CMS physical form)  and Ticket to Play paperwork filled out online. All paperwork and physicals must be tuned in before the first day of tryouts. Tryouts start date will be on August 26th right after school until 6P.M.

1. CMS Pre-Participation Examination Form (signed by student-athlete parent) and NCHSAA Pre Participation Examination form. (Must be Completed by Licensed Physician, Nurse Practitioner or Physician Assistant)
2. Student -Parent Honor Code (signed by student-athlete and parent/legal guardian)
3. Completed green or blue insurance Notice & Release form acknowledging insurance coverage
4. Student/Parent Concussion form (signed by both) Info sheet and SAPLG
5. Copy of participation waiver form and Child Nutrition notification letter (if applicable)
